Your level of care is determined by our specialized care team, who will review your current health, including your medical history, to learn the best way to take care of you and address all of your needs.
Yes. Short term care is best for those recovering from a hospital stay or in need of help to make the transition back home, or to take care of someone when a family caregiver is not available (for instance, while on vacation). If there are limited recovery choices, or if extra time is necessary to move back home, our professionally trained care team can help. Yes, at specific times there is access to transportation for residents.
Yes! Community offers 24-hour skilled nursing care in our care facility.

Yes, although there may be some limitations involved. Please discuss your pet with our staff when coming in for the free tour.
We encourage friends and family to visit their loved ones while at Community Care and Rehabilitation. Our visiting hours are 7:00am - 9:00pm. If wishing to visit outside those time, please give us a call and we can accomodate your visit.
